Utero is the alternate floor of The Womb, and looks exactly the same. The only difference is the variety of enemies and bosses that appear. This floor was added in the Wrath of the Lamb DLC

- "Uterus" is the Latin word for womb. This spelling indicates that the word is being used in the ablative case, suggesting its use as a location as in the phase in utero, "in the womb."
- "In Utero" is also the third, and last, Nirvana album. Things are ending where they began...
