Unlockables are exactly what you would expect: unlockable. They can be several things:

A new item that can be found in future playthroughs .
A new boss, or an alternative boss.
A new character. (Warning: minor spoilers)
A new chapter for future playthroughs.
Difficulty changes.

Unlockables can be gained through doing various tasks, such as shooting enough poop, beating a chapter for the first time, or defeating a boss in a specific way.

Unlockables can be viewed by going to the "Stats" section in the main menu, and then viewing "Secrets" in the left-center of the screen.

The Common Cold	Collectable	Collect two of either Roid Rage, The Virus, and Growth Hormones in one playthrough.
Mom's Contacts	Collectable	Collect Mom's Eye and one other Mom's item in the same playthrough. (Note: Dose not unlock with Mom's Underwear or Mom's Coin Purse and Mom's Eye. Active Items?)
Something from the Future (Steven)	Boss	Defeat bosses in The Basement 42 times.
Something Cute (C.H.A.D.)	Boss	Finish The Caves enough times.
Something Sticky (Gish)	Boss	Finish The Depths 30 times.
Basement Boy	Achievement	Clear the Basement without taking damage.
Spelunker Boy	Achievement	Clear the Cave without taking damage.
Dark Boy	Achievement	Clear The Depths without taking damage.
Mamas Boy	Achievement	Clear the Womb without taking damage.
