Pooters hover around, shooting projectiles at the player when they are close enough.
They spawn in rooms like normal enemies and can also be spawned by The Wretched, Gurdy, Famine, The Duke of Flies, and Mom. Mulligans often spawn Pooters upon death and Swarmers can spawn them after being hit. Pooters are affected by the Skatole and will not shoot if the player has it.

Fat Fly

The Fat Fly is an enemy similar to Pooters that was added in the Wrath of the Lamb expansion. Fat Flies are generally less common than Pooters, but are more likely to be encountered in WotL chapters like Catacombs. They have a chance of replacing regular Pooters spawned in rooms or by enemies.
They have the same health and deal the same damage as the regular Pooter, but shoot two projectiles in a V-pattern similar to that of the Keeper.
Although Fat Flies' projectiles can easily be avoided by going in between them, because they often appear in groups, dodging multiple attacks from them can prove difficult.
Swarmers and The Husk commonly spawn Fat Flies. Monsters that commonly spawn Pooters like Mulligans, Gurdy, Famine, and Mom can occationally spawn them.

- It was once believed that if a Fly was left around Poop, it would eventually take a bite and transform into a Pooter. There is video evidence of a fly turning into a Pooter without any contact to Poop, though, suggesting that the transformation may occur under different circumstances or may be independent of Poop altogether.
- If such a pooter quickly touches another Poop, it is possible for it to change into a Fat Fly or vice versa.
