Not to be confused with the Mask of Infamy boss
It consists of an invulnerable mask, which uses a movement pattern similar to a Knight, and a slow moving heart that will shoot projectiles in the cardinal directions when the player approaches it. The Mask itself is vulnerable to The Necronomicon.
A Heart may spawn as an independent enemy in later levels (such as Utero). It hovers around the room and moves away from the player. When Isaac gets near a Heart, it will shoot blood in cardinal directions.
Tiny Hearts are spawned by the Carrion Queen's Pink Champion Version. They act exactly the same, although they have low HP and move much faster than normal Hearts.
While the Mask is immune to almost all forms of damage, the Hearts are the key to destroying them. Killing the heart will also destroy the Mask it's accompanied with although, if there are multiple Hearts + Masks in the room, it won't necessarily be the mask closest to the heart.
It can be very frustrating if there are 4 of them in one room. You must constantly move and focus on the Heart and avoid the Masks. If there are rocks in the room it can provide cover against the charging masks. Piercing shots are very useful in this situation.
- A boss variant called the Mask of Infamy can be encountered in the first floor in Necropolis. She has a similar mechanic to this enemy. However, the heart will shoot in eight directions and the Mask will not die with the Heart, but continue to fight until it is destroyed by repeatedly shooting its backside.
- Despite both the Hearts and the Masks being able to float, they cannot float over rocks and pits.
- Even though the Mask seems to be immune to all forms of attack, the Blood Rights item can destroy the Mask even if the Heart remains. Same goes for The Necronomicon.
